Personalized Support
Personal care encompasses a wide variety of non-medical services that support maintaining personal hygiene. They include activities of daily living such as dressing, grooming, toileting, showering and bathing.
The level of support is determined during our client intake process and can vary from full hands-on assist to stand by monitoring and anywhere in between.
ALL Caregivers providing personal support services are professionally trained and have PSS certifications. Clients with diagnosis of Alzheimer’s, Dementia, or any other cognitive impairment of any level will receive personal care from a PSS certified Caregiver.
Companion Support
Companion care is a comprehensive approach that assists the elderly with daily tasks in and out of home.
Transportation
Caregivers can provide rides to and from appointments, social gatherings, grocery stores, post offices and many more places’ clients may need to get to.
Light Housekeeping
These services include cleaning of bathrooms, tidying of kitchen and other living spaces. Organizing drawers, cabinets, and closets. Vacuuming, sweeping, and mopping are also provided as part of the plan of care built specific to the client’s needs and requests.
Errand Running
For clients that prefer not to or are unable to safely leave their home our Caregivers can be asked to run daily errands. Grocery shopping, medication pick-up, and post office are just examples of some of the things that can be included in the plan of care.
Meal Preparation
Caregivers can work with clients and their families to help provide nutritional meals to support individual needs. Both food shopping and meal preparation are part of Companion support to provide healthy at home meals that are tasty and nutritional.
Engagement
Research continues to tell us that seniors staying socially active results in a positive impact on their overall health and wellness. Caregivers can engage clients in simple conversation and activities during their time together resulting in strong, meaningful relationships that can last a long time.
SensiCare
A virtual care device that allows us to protect our clients around the clock. SensiCare works by detecting care related events and emergencies via audio. These events are sent in a notification to our office and we share that information with you, so that we can keep you safe. Please take a moment to watch this 1-minute video.
Medication Management
D&C Home Care has the resources to manage and monitor client’s medication needs. We can monitor and order refills when necessary and cue daily intake of medications to ensure that clients are properly taking the medications they need daily.
All medication management is handled by a certified Aging Life Manager. Assisting in scheduling of appointments both in and out of the home through our office staff is also something that can be part of a plan of care.
